Assumption of Risk and Waiver of Liability

I, {name}, hereby agree to the following terms and conditions in consideration of being allowed to participate in activities and use facilities provided by Get Bent.

1. Assumption of Risk  
   I acknowledge and understand that participating in physical activities, classes, personal training, or using gym equipment involves inherent risks, including but not limited to injury, illness, disability, or even death. I voluntarily accept these risks and agree to participate at my own risk.

2. Release of Liability 
   I, {name}, on behalf of myself, my heirs, personal representatives, and assigns, hereby release and discharge Get Bent, its staff, trainers, owners, and affiliates from any and all claims, demands, causes of action, or liabilities of any kind arising from my participation in activities at the gym, including but not limited to injury, loss, or damage to property.

3. Medical Clearance  
   I confirm that I am in good physical condition and have no known medical or health problems that would restrict my participation in physical activities at the gym. I understand that it is my responsibility to seek medical clearance from a healthcare provider if necessary.  

4. Rules and Conduct
   I agree to follow all gym rules, guidelines, and safety protocols while using the facilities. I acknowledge that failure to follow these rules may result in suspension or termination of my membership.  

5. Photography and Media Release 
   I grant permission for Get Bent to use any photographs or video footage taken of me during my participation in gym activities for promotional or educational purposes. I understand that no compensation will be provided for the use of these materials.  

6. Indemnification
   I agree to indemnify and hold harmless Get Bent from any claims, losses, or damages arising from my participation in activities at the gym or use of its facilities.

7. COVID-19 Waiver  
   I understand that despite all precautions, there is still a risk of exposure to COVID-19. I agree not to hold Get Bent liable for any COVID-19-related illness or complications.  

8. Termination of Membership
   I understand that Get Bent reserves the right to terminate my membership if I fail to adhere to its rules, or if I engage in behavior that is deemed harmful or dangerous to myself or others.

Acknowledgment and Agreement

By signing this waiver, I acknowledge that I have read, understood, and agree to the terms and conditions stated above. I understand that by signing, I am waiving certain legal rights, including the right to sue Get Bent in the event of injury, illness, or other claims.

If the participant is under 18 years of age, a parent or guardian must sign this waiver.

This waiver covers all membership-related activities and will remain in effect for the duration of the membership.

Terms and Conditions

1. Membership Duration and Commitment  
   This is a month-to-month membership agreement, which means there is no long-term commitment. The member may cancel at any time with at least 30 days' written notice.

2. Payment Terms
   - The membership fee will be billed on the 1st of each month.
   - Payments are automatically charged via the payment method on file. It is the member’s responsibility to ensure that payment information is up to date.

3. Cancellation Policy
   - To cancel this membership, the member must provide 30 days' written notice before the next billing cycle.
   - If a member fails to provide 30 days' notice, they will be charged for the next month’s membership fee.
   - Cancellation requests should be submitted via email or in person at the gym.

4. Gym Rules and Regulations  
   The member agrees to follow all gym rules, safety guidelines, and protocols set forth by Get Bent. Failure to comply with these rules may result in the suspension or termination of membership without a refund.

5. Liability Waiver 
   By signing this agreement, the member acknowledges the inherent risks of physical activity and agrees not to hold Get Bent, its owners, staff, or affiliates liable for any injury, accident, or loss that may occur while using the gym's facilities. The member is encouraged to consult with a physician before beginning any exercise program.

Acknowledgment and Agreement
